Apply online for Illinois Cares Rx using the IL-1363 Online Form. If you already applied, go online to check your application status.

Please note that if you are asked to file either a Schedule A or P or send any attachments, you MUST fill out the paper version of the Form IL-1363. Download a PDF of the form at www.cbrx.il.gov or call the Senior Helpline at 800-624-2459 (800-544-5304 TTY) to request a copy be sent to your home. You can send written requests to the Illinois Department on Aging at P.O. Box 19003, Springfield IL 62794.

Consider performing a BenefitsCheckUp online screening to determine eligibility for help paying for your prescriptions as well as many other public benefits (no password required).
